Good Faith Estimate for the Costs of Testing

We accept Blue Cross Blue Shield, Aetna, United Healthcare, and Humana for both commercial and Medicare plans. We are no longer taking Cigna.

Testing Materials Fee

Before looking at insurance costs, please note that all patients (except those with Medicaid) pay a Testing Materials Fee of $250.

  • What it is: This is a flat fee that covers the specialized kits, protocols, and administration materials used during your evaluation.

  • Not Covered by Insurance: Insurance companies do not pay for these materials.

  • Out-of-Pocket: You must pay this $250 in addition to any co-pays or deductible costs.

  • Deductible Note: This fee does not count toward your insurance deductible.

Estimating Total Costs vs. Your Share of the Costs

  • We can tell you how much the entire testing costs for you and the insurance company

  • We cannot tell you what percentage your insurance will pay and what percentage you will pay. You need to call your insurance company for that information.

  • If your insurance company does not pay us for any reason, you will be responsible for the charges.

To understand your estimate, identify which type of plan you have below. Remember to add the $250 Materials Fee to these totals.

If you have a Co-Pay Plan:

  • You pay a set dollar amount for every visit. Testing consists of 3 visits.

    • The Math: (Your Co-Pay $\times$ 3) + $250 = Your Total

If you have a Deductible Plan:

Your cost depends on how much of your deductible you have already spent this year:

  • If your deductible is higher than the cost of testing: You may have to pay the entire insurance cost listed in the table below, plus the $250 Materials Fee.

  • If your deductible is lower than the cost of testing: You will pay what you owe toward the deductible, plus your "co-insurance" percentage for the remaining balance, plus the $250 Materials Fee.

  • If you have met your deductible: You only pay your co-insurance percentage (e.g., 20% or 30%) plus the $250 Materials Fee.

Questions to Ask:

  • The Total Cost: "The total combined fee for my testing is the number listed next to your insurance company int he above table. Based on my specific plan, how much of that will I have to pay, and how much will the insurance company pay?"

  • The Plan Type: "Do I have a Co-pay plan (a set dollar amount per visit) or a Deductible/Co-insurance plan (a percentage of the total)?"

  • The Deductible: "How much of my deductible do I have left to pay for this year?"

  • The Co-Insurance: "Once my deductible is met, what is my co-insurance percentage for outpatient psychological testing?"

  • CPT Codes: If the person would like the Procedure codes to do an estimate, you can give them the following information:
